Patient Privacy Protection

NOTE TO PATIENT: We want your informed consent. This means that we want you to understand the services we will provide to you, and what we do with your personal information. If you have a question about any of our policies, please ask.

CONSENT FOR PERSONAL INFORMATION:

In order to provide eye care and optometric services and products, Sheridan Optometric Centre will collect some personal information about each patient. This includes, but may not be limited to contact numbers, birthday, address, OHIP number, medical conditions and medications. This information may be shared in the event that a patient is referred to another health care provider.

Sheridan Optometric Centre may send, without request, some of the following notices and information. If one does not want to receive this information, one may advise Sheridan Optometric Centre of this refusal in writing.

  • Notice when it is time to review one’s eye and vision care needs, including reminder notices for another eye examination
  • Newsletters from Sheridan Optometric Centre
  • Notice of promotions and special offers from Sheridan Optometric Centre

There may be some rare exceptions to these commitments.
